Emotional Support Animals

In an effort to better assist students who are requesting that their Emotional Support Animal (ESA) be permitted to live with them in any WVU sponsored property, OAS has developed separate guidelines for ESA’s which are listed below:

  1. Register with OAS
  1. Submit a completed Licensed Professional Questionnaire - ESA
Please Note: There are individuals and organizations that sell emotional support animal and/or service animal certification/registration documents online. These documents do not convey any rights under the ADA and the Department of Justice does not recognize them as proof of an emotional support animal or service animal. There are no official registries or certifications.
  1. Submit a completed Veterinarian Questionnaire 
  • Submit a completed Veterinarian Questionnaire by:
    • Uploading it when completing the online application 
    • Faxing it to 304-293-8361
    • Bringing it to your initial meeting
  1. Submit an up-to-date picture of your ESA
  • Submit an up-to-date picture of your ESA by:
    • Uploading it when completing the online application 
    • Faxing it to 304-293-8361
    • Bringing it to your initial meeting
  1. Meet with an Accessibility Specialist 
  • Contact the Office of Accessibility Services at 304-293-6700 to schedule your in-person meeting with an Accessibility Specialist. During an initial meeting, specialists will discuss with you: 
    • Submitted documentation
    • Additional documentation, if needed
    • Any other relevant information
Privacy Note: All documentation submitted to OAS is kept separate from an academic record and is considered private under the Family Education Rights and Privacy Act (FERPA) . There are times when the staff of OAS must share information with other faculty and staff on campus to facilitate accommodations in the classroom, residence halls, or transportation.
