Recorded Lecture Agreement Form
Students who create audio recordings as part of their Notetaking accommodations should present this form to faculty when discussing implementation of accommodations. Both the student and faculty should sign this form.
Captioning Equipment Contract
Students who borrow equipment from OAS for the purposes of receiving real-time captioning/transcribing must complete this form.
